SUMMARY:Free Physicals Night - 7th-12th grade
DESCRIPTION:FREE Sports Physicals will take place SUNDAY\, August 9th\, beginning at 5:00 pm – 6:30 pm at Sacred Heart School.  \nALL Students that plan to participate in an extracurricular activity Fall\, Winter\, or Spring grades 7-12 should plan to attend.  If you can not attend Sunday\, you will need a current physical and the athletic forms on file in the SHS main office prior to your first practice. \nStudents and parents ENTER through the West Parking Lot doors with MASK on – sign in and wait on the bleachers in McGremlin Gym.  Students & parents will be given a number based on when they arrive and STUDENTS will be called accordingly to the following stations. \n\n\nEye test \n\n\nWeight & Height  \n\n\nBlood pressure & Pulse  \n\n\nDoctors  \n\n\nOnce exams and forms are completed students will turn their form into the designated volunteer and the originals will be kept in the Main Office at school.  A copy can be made available if needed.  \nPlease EXIT using the Circle Drive Doors when finished. \n  \nParticipation documents including parent permission\, permission waiver\, and medical history are required each year. \nhttps://www.mshsaa.org/resources/pdf/CurrentMSHSAAPre-participationDocumentation-AnnualRequirements.pdf \n   \nA physical exam is required every two years for students participating in interscholastic athletics.  \nhttps://www.mshsaa.org/resources/PDF/Current%20Pre-participation%20Physical%20Evaluation%20-%20PPE-History%20Form-English.pdf \n  \nYou may print and have these completed before you arrive for physicals or we will have forms available at school.   \nBoth forms need to be fully completed & in the school office prior to the student participating in practice.
